Job Summary

A healthcare company is searching for a person to fill their position for a Telecommute RN Care Coordinator in Southeastern Virginia.

Must be able to:

  • Lead Interdisciplinary Care Team meetings
  • Monitor the provision of services as outlined in the Individual Care Plan
  • Manage assigned case load

Qualifications for this position include:

  • Ability to conduct home visits with members
  • Associates in Nursing and Bachelors in Nursing
  • Must be a licensed Registered Nurse in the state of Virginia
  • 1+ years clinical experience with individuals who meet the Managed Long Term Services
  • 1 year with members dependent upon life sustaining equipment for Private Duty Nursing
