Job Summary

A health insurance company is searching for a person to fill their position for a Telecommute Social Work Case Manager.

Core Responsibilities of this position include:

  • Utilizing available community, government, and/or client resources needed to bridge benefits and gaps in care
  • Assisting members to effectively utilize available resources to meet their personal health needs
  • Providing community resources to members seeking solutions to specific social or financial problems

Qualifications for this position include:

  • Requires an MS (at a minimum) in Social Work
  • 3+ years of experience in case management in a health care environment
  • Current unrestricted LMSW or LCSW (or equivalent) license in applicable state required
  • Medicare knowledge
