Job Summary
A healthcare company needs applicants for an opening for a Telecommute State Licensed Registered Nurse Diabetes Care Manager.
Core Responsibilities of this position include:
- Advocating, informing and educating beneficiaries on services and health benefits
- Generating referrals to providers, community-based resources, and resources to assist in goal achievement
- Documenting all encounters with providers, members, and vendors
Must meet the following requirements for consideration:
- Possess an active New York State RN license
- Possess Disease Management experience with Diabetes Educator Certification (CDCES) preferred
- Experience managing member information in a shared network environment helpful
- Experience and knowledge of the relevant product line helpful
- Experience using Microsoft Excel with the ability to edit, search, sort/filter and other Microsoft and PHI systems
