Job Summary

A healthcare agency has a current position open for a Telecommute Support Planner Case Manager in the Montgomery County Area.

Individual must be able to fulfill the following responsibilities:

  • Gathering information to assist in the development of plans of service provided to clients
  • Promoting positive health behaviors to support optimal health and well-being
  • Evaluating client's capacities to assist in the development of plans of service

Qualifications for this position include:

  • This Position includes traveling to meet our clients
  • Ability to conduct research on a computer and other sources
  • Bachelor's degree from a four-year college or university in health/human services or related field
  • Ability to use Microsoft Office, Excel, and PowerPoint
  • Ability to write routine reports and correspondence
