Job Summary
A well-known healthcare company is searching for a person to fill their position for a Telecommute Transitions RN Case Manager in Spokane.
Individual must be able to fulfill the following responsibilities:
- Provide patient education to assist with self-management
- Visit members in the hospital to engage in post hospital program
- Coordinate care and services for members
Applicants must meet the following qualifications:
- Access to reliable transportation and ability to travel to visit members within Spokane, WA
- 5 years' experience as an RN in a clinical setting
- Current, unrestricted RN license in Washington
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Must have a designated work space and access to install secure high speed Internet via cable / DSL in home
