A healthcare company is in need of a Telecommuting Diabetes Education Community Health Worker in Dayton.
Core Responsibilities Include:
- Serving as an intermediary between Health and Social Services to facilitate access to health services
- Removing social, cultural and environmental barriers
- Improving identified health outcomes
Position Requirements Include:
- Requires travel to individual's homes
- One-year experience with community resources within area served
- Effective oral and written communication skills and attention to detail
- Proficient computer skills and familiarity with suite of Microsoft Office Programs
- High School Diploma, GED or higher education level
