Job Summary
An insurance company has an open position for a Telecommuting LTSS Service Coordinator Field Registered Nurse in the Madison Area.
Individual must be able to fulfill the following responsibilities:
- Developing the care plan for services for the member and ensuring the members access to those services
- Identifying members for high risk complications and coordinates care
- Performing face-to-face clinical assessments for the identification, evaluation, coordination and management of members' needs
Applicants must meet the following qualifications:
- Travel within assigned territory in VA is required
- Experience in MS Office suite to include Word, Outlook and Excel
- Current, unrestricted RN license in the state of Virginia is required
- Minimum of 3 years experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities
