Job Summary
An operations management and analytics company has an open position for a Telecommuting Special Needs Program Case Manager.
Core Responsibilities of this position include:
- Performing telephonic case management and care coordination activities
- Performing telephonic assessment in accordance with Health Integrated policy and procedure
- Collecting pertinent clinical information
Skills and Requirements Include:
- Case Management certification within three years of employment
- Current, unrestricted RN or LCSW licensure
- 3 years clinical practice experience
- Practice case management and/or disease management within the scope of their licensure
- 3 years full-time direct clinical or critical care to patients in a medical/surgical or behavioral health setting; or 3 years of experience in applying healthcare criteria or a behavioral health set of criteria
