Job Summary

An operations management and analytics company has an open position for a Telecommuting Special Needs Program Case Manager.

Core Responsibilities of this position include:

  • Performing telephonic case management and care coordination activities
  • Performing telephonic assessment in accordance with Health Integrated policy and procedure
  • Collecting pertinent clinical information

Skills and Requirements Include:

  • Case Management certification within three years of employment
  • Current, unrestricted RN or LCSW licensure
  • 3 years clinical practice experience
  • Practice case management and/or disease management within the scope of their licensure
  • 3 years full-time direct clinical or critical care to patients in a medical/surgical or behavioral health setting; or 3 years of experience in applying healthcare criteria or a behavioral health set of criteria
