Job Summary
A company is looking for a Telehealth Nurse Practitioner.
Key Responsibilities:
- Provide world-class care to patients using data-driven decision-making
- Conduct live telehealth assessments and recommend treatment plans
- Collaborate with telehealth nurses and physicians to coordinate patient care
Required Qualifications:
- 5+ years of Nurse Practitioner experience with a focus on adult chronic care management
- Completion of an accredited FNP program and national certification
- Active NP license in multiple states, willingness to obtain more as needed
- Board Certification and ability to become credentialed with Medicare and Medicaid
- Ability to make evidence-based decisions and adapt to a fast-paced environment
