For-profit health insurance company has an open position for a Teleworking Bilingual-Spanish Social Services Care Manager.
Individual must be able to fulfill the following responsibilities:
- Actively participate in all enrollment activities
- Proactive telephonic outreach
- Coordinate community care and services as deemed appropriate
Qualifications for this position include:
- Masters degree in Social Work
- Three years of care/case management experience
- Knowledge of community health, community resources, and social service agencies
- Exceptional communication and interpersonal skills
- Excellent keyboard and web navigation skills
- Intermediate to advanced computer skills and experience with Microsoft Word and Excel
