Job Summary
Coordinating patient care in a full-time capacity, the Texas Licensed Care Manager LVN will support patients through transitions of care, address barriers to health, and collaborate with interdisciplinary teams to enhance patient outcomes while working remotely with occasional in-person meetings in Houston.
Key responsibilities
- Identify and arrange community services based on knowledge of Medicare, Medicaid, and other programs
- Coordinate patient care through assessment, planning, and implementation while managing discharge plans
- Act as a patient advocate regarding insurance coverage and maintain comprehensive clinical records
Required qualifications
- Active LVN license in the State of Texas
- At least three years of clinical nursing experience, preferably in case management or care coordination
- Knowledge of Medicare, Medicaid, managed care, and community-based resources
- Proficiency with electronic health records (EHRs) and care management systems
- Certified Case Manager (CCM) certification preferred
