Job Summary
A company is looking for a Transplant Nurse Case Manager RN, Medicaid - Remote.
Key Responsibilities
- Make outbound calls to assess members' health status and perform utilization review
- Identify gaps in treatment plans and provide patient education for self-management
- Support members with condition education, medication reviews, and referrals to resources
Required Qualifications
- Current, unrestricted RN license in the state of residence
- 3+ years of RN experience in a hospital, acute care, or direct care setting
- 1+ year of experience working with solid organ or bone marrow transplants
- Ability to obtain Nurse license in other states as needed
- Access to high-speed internet and a dedicated workspace from home
