Required Qualifications

• 4+ years of clinical experience post-residency in complex care, chronic care management, or primary care
• Board certified in Family Medicine or Internal Medicine
• Active state licensure in specified states and ability to obtain additional licenses

• Comfortable using technology and various applications for virtual care delivery
