Job Summary
A health insurance company is filling a position for a Virtual Social Work Case Manager.
Individual must be able to fulfill the following responsibilities:
- Addressing participant's interventions in the management of the participant's chronic condition
- Managing behavioral and psychosocial needs that result in improved clinical and financial outcomes
- Assisting members to effectively utilize available resources to meet their personal health needs
Skills and Requirements Include:
- MS (at a minimum) in Social Work
- AND 3 years of experience in case management in a health care environment
- OR any equivalent combination of education and experience
- Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s)
- Able to ensure patient's effective psychosocial interventions
