Job Summary
A provider of healthcare services is searching for a person to fill their position for a Remote Geriatric Case Management Social Worker in Miami-Dade County, Florida.
Candidates will be responsible for the following:
- Playing an integral part in the coordination of care assessing the members condition
- Providing members with the necessary resources, options, and coordination for a healthy care outcome
- Coordinating and monitoring the care of members and developing a plan of care
Qualifications for this position include:
- Bachelor's degree in social work, psychology, gerontology, human services or related field
- One to three years direct clinical experience
- Two to four years experience with case management or care coordination with geriatric populations
- One to two years experience in a Managed Care Organization
