Job Summary

A company is looking for an Access Center Navigator - Per Diem.

Key Responsibilities
  • Serve as a liaison for referring physicians, patients, and caregivers to facilitate appointment scheduling and service utilization
  • Navigate patients through the continuum of care and coordinate services, ensuring optimal transitions and addressing barriers to care
  • Perform data entry, maintain confidential records, and follow up with patients while collaborating with care teams
Required Qualifications
  • 2-3 years of customer service or call center experience
  • High school diploma or equivalent; associate's degree preferred
  • Healthcare experience with knowledge of medical terminology and managed care preferred
  • Ability to use general office equipment, including a multi-line telephone system
  • Familiarity with databases such as Epic and CRM is beneficial
