Active Care APP Manager

Job Summary

To lead the Active Care Service, the full-time Active Care APP Manager will oversee clinical and operational aspects of a team of Advance Care Providers, ensuring high-quality primary care delivery while maintaining a limited panel of clinical visits in a remote environment.

Key responsibilities

• Provide ongoing coaching and support to APPs, ensuring strong clinical reasoning and member-centered care
• Monitor performance and quality oversight, identifying trends and implementing improvement plans
• Collaborate with primary care leadership to design and implement effective workflows for the Active Care Service

Required qualifications

• Prior primary care clinical background with strong management experience
• 3+ years NP clinical experience in relevant care settings
• Demonstrated experience leading change and supporting clinician development
• Active, unrestricted RN + APRN licensure and national certification
• Fluid usage of the Google suite of products
