Key Responsibilities

Collaborate daily with the Nurse Navigator during the Transitional Phase
Support After Care Operations and coordinate patient referrals
Manage clinical tasks and follow-ups within the Electronic Medical Record inbox and portal platform

Qualifications

2 years of previous clinical care experience
High school diploma or GED
Ability to work evenings, early mornings, and/or weekends
Organized and able to thrive in fast-paced environments
Basic knowledge of medical terminology
