After Hours Nurse Care Manager

Job Summary

A company is looking for an After Hours Nurse Care Manager.

Key Responsibilities:

• Handle inbound and outbound communications related to patient care concerns
• Consult and coordinate with internal and external team members for patient care plans
• Provide health information, coaching, and assistance with medical and wellness issues

Required Qualifications:

• Registered Nurse (RN) with current resident compact licensure in assigned state(s)
• Bachelor's degree, at minimum
• 3-5 years of recent clinical experience
• Experience in call center, triage, consulting, or similar environment
• Disease Management, Case Management, Utilization Review, or Wellness experience
