Job Summary

A company is looking for a Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members.

Key Responsibilities
  • Conduct outreach to educate members about program benefits and facilitate care coordination
  • Assess member status and develop personalized care plans to address their goals
  • Manage a caseload, ensuring timely contact and coordination of care with healthcare providers
Required Qualifications
  • 2+ years of experience in care coordination, case management, or related fields
  • Experience working with Medicaid and/or Dual Eligible patients
  • Remote work experience preferred
  • Experience with documenting case notes in care management or electronic health record platforms
  • Dedicated home office setup for remote work
