Job Summary
A company is looking for a Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members.
Key Responsibilities
- Conduct outreach to educate members about program benefits and facilitate care coordination
- Assess member status and develop personalized care plans to address their goals
- Manage a caseload, ensuring timely contact and coordination of care with healthcare providers
Required Qualifications
- 2+ years of experience in care coordination, case management, or related fields
- Experience working with Medicaid and/or Dual Eligible patients
- Remote work experience preferred
- Experience with documenting case notes in care management or electronic health record platforms
- Dedicated home office setup for remote work
