Job Summary
A company is looking for a Telephonic Care Navigator - Remote in AL.
Key Responsibilities:
- Completes telephonic outreach to members and assists with scheduling provider appointments
- Educates members on gaps in care and connects them with community resources
- Maintains confidentiality of health information and meets quality and performance metrics
Required Qualifications:
- High School Diploma / GED or equivalent work experience
- 1+ years of call center and/or telephonic customer service experience
- 1+ years of professional experience in an office setting using telephone and computer
- 1+ years of healthcare, insurance, social work, or community outreach experience
- Ability to attend 100% of the 3-week training period
