Job Summary

A company is looking for an LPN Care Navigator to provide health coaching and manage patient care remotely.

Key Responsibilities

• Manage a panel of 400-450 patients, conducting monthly outreach and daily calls to provide care plan support
• Collaborate with clinical teams to address patient needs and support care coordination goals
• Advocate for patients by listening to concerns and communicating solutions to their care circle

Required Qualifications

• Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license with a multi-state compact license
• Minimum of 2 years' clinical experience
• Experience with EMR systems and chronic condition management
• Intermediate to advanced computer skills for navigating multiple systems
• Telephonic and virtual communication skills for patient engagement
