Job Summary

A company is looking for a Nutritional Support Coordinator to manage patients' nutritional care through remote counseling and care coordination.

Key Responsibilities

• Manage an assigned panel of 300-350 patients, conducting monthly nutritional outreach and support
• Develop and implement personalized nutritional care plans and educate patients on dietary modifications
• Collaborate with clinical teams to address nutritional needs and maintain accurate documentation in compliance with HIPAA

Required Qualifications

• Bachelor's degree in Dietetics or Nutrition
• Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience
• Experience supporting patients with chronic conditions in remote settings
• 2+ years of Electronic Medical Records (EMR) and computer navigation experience
• HIPAA-compliant home office setup with required internet capability
