Job Summary

A company is looking for a Pharmacist to work remotely in specific states.

Key Responsibilities

• Engage members and schedule pharmacist consultation appointments
• Utilize prescription claims data to provide clinical recommendations and assess medication regimens
• Document patient care notes and participate in departmental meetings to improve clinical programs

Required Qualifications

• Must reside within Louisiana, Tennessee, Alabama, Kentucky, South Carolina, or Minnesota
• Active and in good standing pharmacist license in state of residence
• Bachelor's degree or PharmD required
• 2+ years of pharmacist experience preferred, but new grads will be considered
• Strong knowledge of disease state management and Medication Therapy Management (MTM) services
