Job Summary

A company is looking for a Pharmacist in Alabama.

Key Responsibilities

• Perform and document comprehensive medication reviews and create medication action plans
• Conduct telephonic consultations with medical providers and communicate medication management strategies
• Collaborate with clinical staff to integrate medication management into overall care and participate in interdisciplinary meetings

Required Qualifications

• Bachelor's degree in pharmacy or PharmD
• Active pharmacist license in the state of position
• 3+ years of experience as a practicing pharmacist, preferably in managed care or clinical practice
• PGY1 residency in ambulatory care or related clinical field is preferred
• BPS board certification in pharmacotherapy or related clinical field is desired
