Alabama Licensed RN Case Manager

Job Summary

Working remotely, the part-time Alabama Licensed RN Case Manager will assess patient health status, establish care goals, and collaborate with multidisciplinary teams to address medical, functional, and social barriers to care.

Key responsibilities:

• Assess the health status of members and establish goals to meet their healthcare needs
• Plan, implement, and evaluate responses to the plan of care while collaborating with the multidisciplinary team
• Document patient interactions accurately in electronic medical records and provide education on health maintenance and disease prevention

Required qualifications:

• Current unrestricted licensure as RN in Alabama
• 2+ years of experience as an RN
• Experience in assessing the medical needs of patients with complex behavioral, social, and/or functional needs
• Proven computer skills, including the use of Electronic Medical Records
• Access to reliable home internet and a home office to maintain HIPAA privacy
