Alabama RN Case Manager
Job Summary
To support a growing Medicare and Medicaid care management program, the full-time Remote RN Case Manager will conduct comprehensive assessments, develop individualized care plans, and coordinate care for members with chronic medical and behavioral health conditions.
Key responsibilities
- Conduct comprehensive assessments of members' healthcare needs and develop individualized care plans
- Provide ongoing care coordination and connect members with healthcare resources and support services
- Document care management activities accurately in the electronic health record while meeting regulatory requirements
Required qualifications
- Active and unrestricted RN license in Alabama or a Compact RN license in the state of residence
- Associate's degree in nursing (ASN) with a minimum of 3 years of nursing experience
- At least 2 years of case management, discharge planning, or home healthcare coordination experience
- Proficiency in Microsoft Office Suite
- Previous remote work experience
