Alaska Health Coach (NBC-HWC)
Job Summary
Providing one-on-one coaching to members managing chronic health conditions, the full-time Alaska Health Coach (NBC-HWC) will develop behavior-based coaching programs and healthcare navigation support while working remotely from Alaska.
Key responsibilities
- Conduct initial health assessments and document member progress towards health goals
- Educate and empower members using evidence-based guidelines to support disease prevention and self-management
- Collaborate with healthcare teams to enhance member experience and improve health outcomes
Required qualifications
- Associate degree, preferably in a healthcare-related field
- Completion of a training program approved by the National Board for Health and Wellness Coaching (NBHWC)
- At least one year of experience in a health coaching-related field
- Specialty credential such as NBC-HWC, RDN, LCSW, or LPC is a plus
- Solid understanding of health behavior change theories and their practical application
