Alaska Licensed Care Navigator

Job Summary

A company is looking for a Care Navigator with an Alaska nursing license.

Key Responsibilities:

• Complete phone consultations with patients enrolled in care management and remote patient monitoring programs
• Educate patients about their health conditions and empower them to manage their chronic conditions
• Maintain accurate and up-to-date patient records and respond to remotely transmitted patient data

Qualifications:

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management
• Reliable internet connection and HIPAA-compliant work area
• Proficiency with electronic health records (EHR) and virtual communication tools
• Additional state licenses may be required and reimbursed
