Job Summary

A company is looking for a Registered Nurse Disease Manager for remote positions.

Key Responsibilities

• Perform assessments and coordinate care for dependents of Active Duty and eligible retirees
• Provide education and training related to disease management
• Work remotely in a high-volume environment

Required Qualifications, Training, and Education

• Bachelor's Nursing Degree
• At least one active state nurse license, preferably compact state licensure
• A minimum of 2 years' experience as a Registered Nurse Disease Manager, ideally in a remote setting
• Preferred certifications in disease-management areas (e.g., CCP, CHC, CDE, AE-C)
