Job Summary

A company is looking for an Alaska Health Coach (NBC-HWC).

Key Responsibilities

• Provide one-on-one coaching to members managing or at risk for chronic health conditions through various communication channels
• Educate and empower members in setting and achieving SMART health goals using evidence-based guidelines
• Partner with healthcare teams to improve member experience, outcomes, and reduce avoidable costs

Required Qualifications, Training, and Education

• Associate degree, preferably in a healthcare-related field
• Completion of a training program approved by the National Board for Health and Wellness Coaching (NBHWC)
• At least one year of experience in a health coaching-related field preferred
• Experience working with individuals managing chronic conditions is preferred
• Strong understanding of health behavior change theories and their practical application
