Job Summary
To support patients enrolled in chronic care management and remote patient monitoring programs, the full-time Alaska Licensed LPN Care Navigator will provide education, support, and communication with patients while collaborating with their care teams in a remote setting.
Key Responsibilities:
- Complete phone consultations with patients, providing support and education about their chronic conditions
- Educate patients on health management strategies and assist them in achieving goals in line with their care plans
- Maintain accurate patient records and document all interactions according to protocol
Required Qualifications:
- Current, valid, and in good standing Alaska Nursing License (LPN/LVN)
- 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management
- Proficiency in using electronic health records (EHR) and care management software
- Reliable internet connection and proficiency with virtual communication tools (e.g., Zoom, Slack)
- Empathy and a patient-centered approach to care
