Job Summary
To support a growing care navigation program, the full-time Alberta Licensed Healthcare Intake Coordinator will conduct proactive telephonic outreach, complete clinical intake questionnaires, and ensure patients transition smoothly into active care while working remotely.
Key responsibilities
- Manage structured outbound referral queues to engage prospective program participants with warmth and professionalism
- Guide patients through program benefits and complete standardized clinical intake questionnaires in real time
- Build rapport with older adults to address hesitations and facilitate appointment scheduling while adhering to compliance protocols
Required qualifications
- Holds a Medical Office Assistant (MOA) Certificate/Diploma or equivalent verified Canadian healthcare administration experience
- 1+ years of patient-facing healthcare experience involving medical terminology and clinical data collection
- Proven experience in high-volume outbound phone environments related to clinical outreach or patient scheduling
- Comfortable navigating multiple software systems and electronic health records (EHRs) during live conversations
- Possesses a "builder" mentality with adaptability to shifting processes and ambiguity
