Ambulatory Care Pharmacist - OH, MI, IL

Job Summary

To support a growing healthcare team, the full-time remote Ambulatory Care Clinical Pharmacist - OH, MI, IL will manage Comprehensive Medication Management (CMM) and collaborate with primary care providers and specialists to optimize chronic disease management and improve health outcomes.

Key responsibilities:

• Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams
• Develop medication management action plans to achieve member self-management goals and clinical outcomes
• Communicate with internal and external providers to ensure alignment on medication-related goals and care plans

Required qualifications:

• Active pharmacist license in at least one of the following states: MI, IL, or OH
• Eligibility and willingness to obtain Ohio licensure upon hire
• PGY1 pharmacy practice residency and PGY-2 Ambulatory Care Pharmacy Residency, or a minimum of 2 years' experience as a clinical pharmacist in an ambulatory care setting
