Job Summary
To support a growing telehealth initiative, the full-time Arizona Licensed Advanced Practice Provider (NP or PA) will conduct virtual assessments, manage prescription refills, and collaborate with a cross-functional care team to deliver innovative chronic disease management remotely.
Key responsibilities
- Conduct thorough assessments and develop individualized treatment plans for patients with chronic conditions such as hypertension and diabetes
- Manage a prescription refill inbox and participate in centralized results management workflows for timely patient care
- Utilize digital tools and provide feedback to improve clinical workflows and patient communication
Required qualifications
- Active and in-good-standing RN and APRN licenses for Nurse Practitioners or an active and in-good-standing Physician Assistant license
- Minimum of three years of outpatient clinical practice experience, with a focus on chronic disease management
- At least two years of experience in primary care or a subspecialty related to hypertension and lipid management
- Ability to obtain licensure in multiple states within 90 days of hire
- Experience or strong interest in virtual care and digital health technologies
