Arizona Licensed Behavioral Health Clinician

Job Summary

A company is looking for an AZ Licensed Clinician: LCSW in Behavioral Health and Family Medicine.

Key Responsibilities

• Provide customized care and treatment plans for patients in assigned Care Pods
• Coordinate behavioral health care with primary care providers and monitor patient symptoms
• Document patient progress and treatment recommendations in Salesforce and electronic health records

Required Qualifications and Education

• Master's Degree in Social Work, Counseling, or a related field
• Licensed to practice in Arizona as an independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or equivalent)
• Ability to complete cross-licensing in Montana if required
• Experience with evidence-based treatment approaches is highly preferred
• Fluency in English; proficiency in Spanish is highly preferred
