Arizona Licensed Care Coordinator

Job Summary

Managing a panel of 400-450 patients, the full-time Arizona Licensed Care Coordinator will conduct monthly outreach and health coaching, collaborating with clinical teams and advocating for patient needs in a remote, HIPAA-compliant workspace.

Key responsibilities

• Conduct 50-60 daily outbound calls to patients for care plan support and health coaching
• Collaborate with clinical teams to address patient clinical and social needs
• Document all patient interactions in real-time while maintaining HIPAA compliance

Required qualifications

• Must hold a nationally recognized medical assistant certification or be a Certified Pharmacy Technician
• Minimum 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum internet speed requirements
• Must be legally authorized to work in the United States
