Job Summary
To support patient care initiatives, the Arizona Licensed Care Coordinator will monitor patient health, coordinate with providers, and manage remote monitoring technology in a hybrid or remote capacity, following an initial week of training in Scottsdale, AZ.
Key responsibilities
- Coordinate patient enrollment, assist with device setup, and educate patients on remote monitoring technology
- Monitor and analyze patient data from remote devices, identifying trends and escalating issues to healthcare providers
- Collaborate with healthcare professionals to optimize patient care and ensure accurate documentation in electronic health records
Required qualifications
- At least 1 year of clinical work experience as a Medical Assistant
- At least 1 year of EMR experience, preferably with eClinicalWorks
- Medical Assistant Course Completion Diploma or CMA designation required
- High school diploma or GED equivalent
- Strong customer service skills and a high level of professionalism
