Arizona Licensed Care Coordinator
Job Summary
Managing the member experience from trip intake to completion, the full-time Arizona Licensed Care Coordinator will monitor transportation trips, provide member support, and ensure successful trip outcomes while working remotely.
Key responsibilities
- Act as a liaison with health plan clients, members, and partners to ensure a superior experience for members needing extra care
- Monitor and confirm transportation trips, handle trip recovery, and resolve issues related to trip accuracy and member appointments
- Document escalations and action plans, maintaining compliance with KPIs and contract requirements
Required qualifications
- High school diploma or G.E.D. equivalent
- 9 months of experience in a Customer Care Representative role or similar experience in health care case management
- Proficient computer skills, including Microsoft Outlook, Word, and Excel
- Ability to handle escalated situations with an analytical thought process
- Familiarity with medical terminology preferred
