Arizona Licensed Case Management Nurse

Job Summary

Providing proactive telephonic case management, the fully remote Arizona Licensed Case Management Nurse will support members with complex or chronic conditions by assessing needs, developing individualized care plans, and advocating for appropriate services, while working four 10-hour shifts per week.

Key responsibilities

• Identify members for case management and conduct comprehensive assessments of their medical, behavioral, and social needs
• Develop and coordinate individualized care plans, facilitating communication among patients, providers, and health plans
• Provide telephonic case management for high-utilization patients and support utilization review activities as needed

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in good standing
• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical nursing experience, with a focus on OB, maternity, or related fields
• Strong knowledge of chronic disease management and experience managing complex patient populations
• Ability to work independently in a remote environment and manage confidential information in compliance with HIPAA
