Arizona Licensed Case Management Nurse

Job Summary

Supporting members with complex oncology and transplant-related needs, the fully remote Arizona Licensed Case Management Nurse will conduct assessments, develop individualized care plans, and advocate for appropriate services while working four 10-hour shifts per week within standard operating hours.

Key responsibilities

• Identify and assess members who would benefit from case management using clinical data and referrals
• Develop and coordinate individualized care plans, facilitating communication among patients, providers, and health plans
• Provide telephonic case management and advocate for timely, cost-effective care while ensuring compliance with regulations

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in good standing
• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical nursing experience, including current or recent oncology and transplant nursing experience
• Strong knowledge of chronic disease management and experience managing complex patient populations
• Ability to work independently in a remote environment and manage confidential information in compliance with HIPAA
