Job Summary

Seeking a compassionate and skilled Arizona Licensed Clinical Social Worker, the remote position will provide comprehensive psychosocial assessments, counseling services, and care coordination for patients and families within a virtual medical setting, focusing on patient-centered care in a healthcare environment.

Key responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs
• Collaborate with healthcare professionals to ensure holistic patient care and coordinate community resources
• Provide emotional support and counseling to patients, educating them about medical conditions and treatment options

Required qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program
• Current Arizona state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent
• Minimum two years of clinical social work experience preferred
• Experience in healthcare/medical settings strongly preferred
• Availability to work standard business hours (9 AM - 5 PM) in Pacific Time
