Job Summary

A company is looking for a Complete Care Specialist Arizona Residency.

Key Responsibilities

• Develop and implement a system of care for members, ensuring services meet their needs
• Identify and establish partnerships with stakeholders and community programs to support members
• Provide training and guidance on serving AHCCCS Complete Care members while collaborating with internal and external teams

Required Qualifications

• Bachelor's degree or certification in Social Work or a relevant field
• Four or more years of experience in a relevant role
• Proficiency in Microsoft Office and experience with databases/programs
• Ability to analyze and solve problems effectively
• Experience in developing and implementing goals
