Key Responsibilities

Initiate and participate in multidisciplinary discharge rounds as needed
Conduct telephonic outreach to high-risk members for post-discharge and disease management assessments
Track and follow high-risk members to prevent hospital admissions/readmissions

Required Qualifications

Active RN License
5+ years of experience in the healthcare/health insurance industry, with a preference for 2+ years in case management
Strong clinical documentation skills
Capacity to support program development and change
