Arizona Licensed Discharge Care Manager RN

Job Summary

A company is looking for a Discharge Care Manager Nurse RN, Remote in Mountain or Pacific Time Zone.

Key Responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Coordinate care post-discharge in collaboration with providers and members
• Document discharge activities and participate in team meetings and education discussions

Required Qualifications

• Bachelor of Science in Nursing
• Current, unrestricted Compact RN license in the state of residence
• 5+ years of recent experience in an acute inpatient hospital setting
• 2+ years of experience in discharge planning and/or case management
• Experience with multiple health insurance products and regulatory requirements
