Arizona Licensed Hospice RN

Job Summary

Seeking an Arizona Licensed Hospice RN, the full-time remote position will manage palliative care triage, hospice coordination, and patient education while collaborating with care teams to enhance patient quality of life.

Key responsibilities

• Triage urgent palliative care needs and provide clinical support to regional palliative physicians
• Facilitate hospice referrals, educate patients and families, and maintain communication with hospice agencies
• Conduct patient care tasks and support advance care planning documentation and discussions

Required qualifications

• Graduate of an accredited School of Nursing
• Currently licensed as a Registered Nurse
• 2+ years of experience in palliative-focused care, geriatric home-based care, or hospice care
• Ability to travel to assigned markets up to 20%
• Familiarity with Microsoft Office and web-based applications
