Arizona Licensed RN Case Manager

Job Summary

Managing a panel of members with complex medical and behavioral needs, the full-time Arizona Licensed RN Case Manager will engage members telephonically to conduct comprehensive needs assessments, develop person-centered care plans, and collaborate with care teams while working remotely from Arizona.

Key responsibilities:

• Engage members to complete comprehensive needs assessments, addressing medical, behavioral, and socioeconomic factors
• Develop and implement care plans to manage chronic health conditions and promote health and wellness
• Advocate for members and coordinate care transitions following hospital visits or admissions

Required qualifications:

• Current, unrestricted independent licensure as a Registered Nurse in Arizona
• 4+ years of clinical experience
• 1+ years of experience with MS Office, including Word, Excel, and Outlook
• Must reside in Arizona
