Arizona Licensed RN Case Manager

Job Summary

To enhance patient care and health outcomes, the full-time Arizona Licensed RN Case Manager will manage care transitions, high-risk patient coordination, and referral processes while working remotely within the U.S.

Key responsibilities

• Collaborate with a Care Team to support patient engagement and care coordination activities
• Conduct post-discharge outreach and facilitate communication with primary care offices
• Guide patients in accessing appropriate specialist referrals and coordinate appointments for high-risk patients

Required qualifications

• Bachelor's degree
• Current, active, unrestricted Registered Nurse (RN) license in a Compact License state within the US
• 3+ years of experience in Case Management and/or Care Coordination
• Advanced proficiency in Microsoft Office, particularly Excel and PowerPoint
• Ability to work 1 late shift per week (10 am - 7 pm), with standard hours from 8 am - 5 pm
