Arizona Licensed RN Case Manager

Job Summary

Providing telephonic complex case management, the full-time Arizona Licensed RN Case Manager will coordinate care for adults with complex healthcare needs, partnering with members, caregivers, and providers to facilitate access to necessary services and resources in a remote setting.

Key responsibilities

• Manage individual patients across the healthcare continuum to achieve optimal clinical and operational outcomes
• Collaborate with interdisciplinary teams to develop and communicate effective patient discharge plans
• Evaluate medical necessity and appropriateness of care while resolving barriers to effective patient care

Required qualifications

• Current Registered Nurse (R.N.) license in Arizona
• Bachelor's degree in case management or healthcare
• 3-5 years of clinical experience in care management or acute care settings
• Knowledge of community resources and resource/utilization management
• Basic Life Support (BLS) certification for acute care settings
