Arizona Licensed RN Disease Manager

Job Summary

A company is looking for an RN Disease Manager - Remote.

Key Responsibilities

• Provide Disease Management services to insurance beneficiaries
• Collaboratively assess, plan, implement, coordinate, monitor, and evaluate beneficiaries' health and psychosocial needs
• Develop care plans in collaboration with providers and assess effectiveness to improve health outcomes

Required Qualifications

• Current, unrestricted RN licensure in the state of residence
• Ability to pass a Department of Defense background investigation
• Capability to obtain multi-state licensure if residing in a NCL state
• Minimum of three years clinical experience
• Experience in case management, care coordination, or discharge planning in relevant healthcare settings
